How to Fill Out Form WH‑347 (Rev. January 2025)

The U.S. Department of Labor released a revised WH-347 in January 2025. The legacy form expired on September 30, 2026, so use the revised version for your weekly certified payrolls. This guide walks through the form section by section.

The header

  • Project name and project or contract number.
  • Certified payroll number — starts at 1, one per week, with no gaps.
  • Business name and address of the contractor.
  • Project location.
  • Wage determination number(s), including modification numbers.
  • Week ending date.
  • The prime / sub box and the final payroll box.

Page 1: the worker table

  • 1A Entry no. — use the same number for a worker who appears on several classification rows.
  • 1B–1D Name of the worker.
  • 1E Identifying number — the last 4 digits of the SSN or an employee number. Never the full SSN.
  • 2 J or RA — journeyworker or registered apprentice (add the apprentice level).
  • 3 Classification — taken from the wage determination.
  • 4 Hours per day, on separate straight-time (ST) and overtime (OT) rows.
  • 5 Total hours.
  • 6A Rates — the ST and OT rates actually paid. Do not include cash in lieu here.
  • 6B Total fringe credit = total hours × hourly plan credit.
  • 6C Cash in lieu = total hours × hourly cash amount.
  • 7A Gross for this project and 7B gross for all work.
  • 8 Deductions — tax withholdings, FICA, other (describe in remarks) and total.
  • 9 Net pay for all work.

Page 2: Statement of Compliance

Enter the certifying official. Of the six statements, 1, 2, 3 and 6 are always checked. Statement 4 applies when you employ apprentices and comes with the apprenticeship program table. Statement 5 applies when you pay fringe benefits and comes with the hourly credit table. Add any additional remarks.

Sign page 2 in wet ink and add your phone and email. Falsification can lead to prosecution under 18 U.S.C. 1001 and debarment.

Overtime

On contracts over $100,000 (covered by CWHSSA), hours over 40 in the workweek are paid at least 1.5 times the base rate. The overtime premium does not apply to fringe benefits.

Record keeping

Keep payroll records for 3 years after the contract is completed.
